Informed Consent
1. You acknowledge that in any physical activity there are health risks, and by participating you put yourself at risk of sports related injuries (muscle & joint injuries, discomfort, and muscle soreness) and that you are free to stop participating if you feel pain. You acknowledge that no one else can be held responsible for decisions effecting your health besides you.
2. You consent that you are in good medical health as deemed by a physician and by participating you are not putting yourself at high risk of any cardiovascular, or serious muscle/joint health complications.
3. You agree that ‘Bruce Contant’ is not responsible for any damages you may suffer from participating in this program. These damages include (but are not limited to): theft of personal property, personal injury due to equipment malfunction, personal injury due to environmental hazards or physical injuries/health conditions.
4. You acknowledge that ‘Bruce Contant’ reserves the right to withdraw you from the program at any time if you do not abide by the ‘Terms and Conditions’ of this agreement and those set by the ‘City of Ottawa’ & 'Ottawa Carleton District School Board' in their lease agreement.
5. Refund(s) must be requested within 2 weeks of the program start date: i) you will be refunded the ‘per night’ equivalent of the remaining nights , less 15%, ii) your refund will be issued within 10 business days.
6. Skill Assessment: if you DO NOT meet the skill requirements then you will be offered a credit or refund.
7. You agree to refrain from using profanities. You agree to treat other participants with respect and to discuss any problems with the instructor privately if an issue can not be resolved.
8. You agree to make your best effort to arrive on-time ready to play, dress accordingly (shorts, track pants, running shoes) and to bring a water bottle.
Player Responsibilities
1. The success of our group depends on having 24 (minimum) on each night. Therefore, you will be required to send an email or mark yourself absent on the shared attendance sheet. Failure to indicate an absence on more the 3 occasions may result in your spot being given to another player.
2. Show up on time:
- failure to arrive (ready to play) prior to group drills / warm-up on a regular basis may result in your termination as a participant.
- late arrivals should warm up on their own until group drills are complete, or until the game in progress is completed.
3. Clearly identify your volleyball
- volleyballs are provided and not required, however, if you bring your own ball, please write your name on it.
4. Our time is precious, don't waste it by doing the following:
- calling a ball in the court when you can easily retrieve it or kick it away without affecting the play (and it's not risking anyone's safety)
- play within your skill level (hitting hard is fine if you have a good percentage of the ball landing in the court).
- identify any weaknesses that lead to 'many errors' and seek help.
